This PR introduces the following:
Add the test suite itself to coverage reports.
(Several files in the test suite contain code that isn't getting executed.)
Remove the doctests, which are demonstrably no-ops.
Remove the document building tests, which do not introduce unique testing and contribute nothing to total code coverage.
Reformat
tests/test_create.py, including running black/isort/pyupgrade against the file.
