Allow customization of 'Create Log Entry' view

Currently when adding a new log entry there are many fields that can be changed, however not all logbook clients will have a sensible way of implementing these. Would it be possible to configure what is shown in this view?

At Diamond we would want to be able to add a 'Title' field, and remove the 'Level' field, however I imagine that these requirements vary widely between sites.

At Diamond we would want to be able to add a 'Title' field,

SNS would also prefer a 'title'.
We currently have to treat the first line of the text as the entry's title, but this has not found user acceptance and resulted in mangled log entries.

Parts of this view are configurable, for example at nsls2 I have a separate plugin which contributes a tab to attach shift information to log entries.

After looking at NSCL logbooks we saw that over 80% of the log entries had no titles or had titles which were duplicates of the first line of the entry.
NSLS2, FRIB, ITER, NSRRC and CSNS don't need titles..

Anyway...We can look into creating new preferences that will make the entire widget configurable however that makes it harder to maintain and look nice.
