x86 MMU: implement build-time tool to create boot page table

We want to establish a page table at boot without having to spend CPU cycles setting it up during kernel initialization. Create a build tool which will establish a page table at boot based on the contents of a special data section.

Sub-tasks will enumerate the specific tasks for this effort.
