Splitted the code into 'plugin' and 'autoload' to make MiniSnip even lighter.

1. Moved the function definitions to autoload.
2. Renamed vim-minisnip.vim to minisnip.vim (as is the usually-followed naming style; further, this is also consistent with the already existing doc/minisnip.txt).
