Fix some higher-order (?) ParamSpec usage

This PR just makes my own ParamSpec usages valid (I added a test). I probably need to do another scan of the issues to make sure I didn't fix anything else (cause I did my scan through before a couple more changes in this branch):

• Fixes User-defined TypeGuard for Callable is coroutine function has errors #13710
• Fixes ParamSpec parameters don't match __init__ parameters of class  #12986
• Fixes Incompatible type when using ParamSpec #12169
• Fixes Generic types that use ParamSpec aren't callable #11846
• Fixes PEP-612 ParamSpec with high-order function error. #12595
• Fixes Decorator factory function typing  #15037
• Fixes Regression in 1.2.0: callback protocol with Callable and Concatenate #15065
• Fixes (🐞) Silent crash regression in 1.2 with complex Concatenate return value #15073

It feels like most of these are higher-order so I claim that this helps with that, but I'm not sure. Hopefully mypy primer is positive! I still need to add some regression tests.

Keep in mind that some of the changes are based on just changing things that seemed invalid when I was single-step debugging.
