VLAN can not assigned to Site (UI issue)

Steps to Reproduce

1. Add a new VLAN
2. Try to assign it to a Site by selecting the tab
3. No controls are shown (Style is "display: none;", set by javascript)

Expected Behavior

One can assign a VLAN to a site

Observed Behavior

UI elements not shown
