Incorrect Form in IP Address Assignment Template

Steps to Reproduce

1. From a device's interfaces tab, click the Assign IP Address (+) button
2. Click the Assign IP tab

Expected Behavior

From the Assign IP view, the object create buttons should be replaced by a Search button. When clicked, the search results should be displayed inline.

Observed Behavior

The rendered form is not a search form, and when "Create" is clicked, the page is reloaded.
