Custom Fields header is missing from create forms

Steps to Reproduce

1. Create a Custom Field for dcim > Device Role ('Class' in the example below)
2. Click on the create form for a new Device Role

Expected Behavior

The custom field should have the typical Custom Fields header above the created custom field and associated custom field group (if any).

Observed Behavior

The Custom Fields header is missing.
