…ard focus (now can access toggle buttons via tab-key, and switch state via space) - toggle buttons now preserve the inputs title attribute

my change preserves the title attribute of the underlying input tags (it would probably be even better to even copy the data-* attributes which are used by the default bootstrap tooltip functionality).
and the little handle bars now are real buttons so that these are focusable. now forms using bootstrap-toggle can be entirely filled by keyboard only.
