For most incoming bug reports, its important for us to know how people set up their SDK, specifically, which init options they use. This PR adds such an input field to the "Bug" issue template. I decided to keep it optional for the time being as it doesn't apply to 100% of bug reports and some people already add this to the "Steps to Reproduce" section.
