Adds a runnable version of the getting started example and applies a handful of unrelated bug fixes and cleanups.

Run as follows:

cd skipruntime-ts
make bunbuild
bun run examples/groups.ts &
bun run examples/groups-server.ts &
bun run examples/groups-client
