HTML Output faulty (styles not applied)

It seems the html output is faulty. As in #499, this issue is verifiable via the examples (namely, example 1).

It seems the necessary styles are there...

But are not applied to the elements. Also, the "inline" elements miss further splitting into span tags, to allow the necessary inline styling.

Happens in 0.12, master and develop branch.
