Seems to be a new feature going through a beta?

The actual body comes through like this: (So the app handles it alright but we can do better!)

The original line to diff with is currently the line the comment is attached to.

Is there an API for this? Or is it just custom markdown parsing?

Yea it's not API driven (even on GitHubs side from what I can tell) they just specify the language in the markdown code block as "suggestion" (if you edit my post you can see the exact format) and then it's basically diffed against the line the comment is attached too
