`allOperationIds` does not consider webhooks

This was by design originally but it was also a decision made when webhooks weren't a document property rather than a decision made because webhooks shouldn't be considered. 

Semantically, I'm considering this omission a bug because "all operation ids" should mean _all_ of them.
